Our Story
Our journey is rooted in deep personal experiences where we've witnessed immediate family members struggle with the complexities of care coordination. After stepping in to coordinate their care, it was clear that getting proper post-discharge care was critical in preventing complications, and having support to coordinate it lifted an emotional burden during a vulnerable time.
We’ve also heard countless stories of patients whose recovery was impacted due to gaps in coordination. Patients who ended up in the ER because home health services missed an appointment. Or patients who had to be re-admitted because they weren't shown how to properly administer their medication.
Today, less than 10% of patients receive transitional care support from their health system. We founded Fluent Healthcare to change this paradigm.
